PFO complete a Risk assessment form before all of their events.

You can find our guidelines for dealing with an incident/accident and accident report forms here. You can also find a generic Junior Medical Form here. This may be needed for club training, trips to events etc.

There is also a document outlining the funding that PFO will provide for members of the NWJS and what juniors have to do to earn and claim the funding.
